The Reason

The system is not broken by accident. It was built in pieces. Separate agencies, separate forms, separate waiting lists, separate people asking you to explain your life from the beginning, again.

The people who need the most support spend the most energy just trying to connect the dots. Fragmented systems produce fragmented outcomes.

We exist to end that.

We are something that didn't exist before — not a better version of what's already here. We are the blueprint for what integrated support actually looks like.

Our work is affirming, culturally grounded, and responsive to the communities we serve, including LGBTQ+ people and communities most impacted by barriers to care, housing, income, education, and support.

ABOUT

Our Founder

We’re led by our Founder & Executive Director, Khalil Noah Scott.

 

Raised in group homes, foster homes, and behavioral health facilities. Adopted by his grandmother at sixteen. A survivor of the criminal legal system. Living with HIV. Homeless as an adult in 2024.

 

Every Pathway we offer is one he has needed. He did not study fragmented systems from a distance — he lived inside them, and that gave him a precise picture of exactly what should have existed and never did.

 

We’re not a reaction to a broken system. We’re the blueprint for what the system should have been.
